What information is collected
The review form asks for a test name, the service, a test telephone number or email address and, optionally, an indicative date and time window. It records that the privacy information was read and the technical data needed to manage and protect the request. Do not enter real personal data in this review version.
There are no fields for attaching documents or describing medical history. Do not send a DNI, NIE, diagnoses, reports or health data through the form. Information needed for an examination will be handled through the channel indicated by the centre.
Purpose and legal basis for processing
Contact details are used to respond to your request and coordinate a possible visit. This communication is in response to your request for information or for steps before a service is provided.
Technical security checks aim to prevent abuse and keep the site working. The form does not include a marketing subscription and is not used to make automated medical decisions.
Access, hosting and external services
This version's form is used for review: it requests test data and stores it in the site's management inbox. It does not automatically send messages to the centre or confirm an appointment. To arrange a real visit, use the telephone, email or WhatsApp details published in the contact section. The inbox is protected and access is limited to authorised people.
The site is hosted with OpenAI Sites on Cloudflare infrastructure. These services support its technical operation and security. If you open WhatsApp or a map, you leave for an external provider subject to its own terms and privacy policy. No advertising tracking has been added to the pages.
Retention of requests
Data is retained to manage the request and its follow-up. Once the request is closed, the controller's policy sets a maximum of six months, unless there is a legal retention obligation or it is necessary to deal with a claim. This policy requires management by the controller; the site does not announce automatic deletion.
You can request information about retention or ask for deletion through the controller's channels. This information concerns contact requests; it does not describe the retention of a medical record.
Your rights
You can request access, rectification and deletion and, where applicable, object to or restrict processing or request data portability by contacting info@centromedicosierrablanca.com. Explain which right you wish to exercise and provide a means of receiving a reply. Additional identification information will only be requested if necessary.
If you believe your rights have not been addressed, you can contact the Spanish Data Protection Agency. The AEPD website provides information about these rights and complaint procedures.
